The following is a list of acceptable proofs of residence:

*  property tax receipt record or deed

*  apartment or home rent receipt or lease

*  utility bills or deposit receipts (water, gas, telephone, power, cable, trash)

*  income tax records (copy of check/correspondence) from IRS to parent/guardian

*  certificate of residence/enrollment card (must be notarized)

*  other official proofs:  check or correspondence from Social Security Office, Department of Human Resources, Food Stamp Office,

         or Temporary Assistance to Needy Families

*  bank records (checking/savings/loan statement, investment certificate, bank correspondence)

*  tax records or receipts (business tax records)

*  employment records (company check/verification from employer)

*  two (2) major charge accounts (counts as 1 proof)
